Temporary Abdominal Closure Techniques


Open abdomen treatment (OAT) is increasingly used, most often to prevent intra-abdominal hypertension (IAH) and abdominal compartment syndrome (ACS) after emergency abdominal surgery. The goal of temporary abdominal closure (TAC) techniques no longer is abdominal coverage alone, but fluid control and facilitation of early fascial closure are now important aspects. Various methods are available, but negative pressure therapy seems to be best suited to achieve these goals. Fascial approximation techniques prevent lateral retraction of the abdominal muscles and can be combined with TAC techniques. Mesh-mediated vacuum-assisted wound closure is emerging as one of the most promising approaches for OAT. In the intensive care unit, continued attention to IAH/ACS and measures to prevent or treat these conditions is imperative.

This was first introduced by Colombian surgeons, who used a simple sheet cut out of a 3-L saline irrigation bag (the so-called "Bogot� bag" or "Bolsa de Borraez") that is sutured to the fascia or the skin (Fig. 2). Silicone sheets can be used as well. Various meshes of different material fall into this category; these can be absorbable or nonabsorbable meshes that are sutured to the fascia; polypropylene (Marlex, Prolene, Surgipro), polyglycolic acid (Dexon), polyglactic acid (Vicryl), and PTFE (Gore-Tex) have been used (Fig. 3). Biological meshes such as those manufactured from human or porcine dermis have been used mostly for reconstruction of the abdominal wall at a later stage, but these can also be used for temporary cover; no large-scale experience has been reported so far. Each mesh has specific characteristics and disadvantages that fall beyond the scope of this review. A prosthesis can also be used to progressively approximate the fascia by reducing the size. Other prostheses have been developed specifically with this purpose, e.g., the Wittman patch(TM) (NovoMedicus, Nokomis, FL) (or "artificial burr") that consists of two sheets of Velcro.Indications for Open Abdomen Treatment and Temporary Abdominal ClosureTemporary Abdominal Closure Techniques

Clinical experience confirms that NPT in this setting is safe and despite concerns for the development of intestinal fistulas, the reported incidence in a recent review is the lowest of all TAC techniques.5 The latest version of this dressing, the ABThera(TM) (Kinetic Concepts, Inc., San Antonio, TX), has "arms" that make it easier to insert and also improve the application of the negative pressure across the surface of the abdomen. Dog Hair And Dog Dander? Which Vacuum Is Best For Shedding Pets?


We tend to adore our dogs or cats. They shed year round, and that creates problems when vacuuming your home.


Historically, OAT has been used primarily in patients with abdominal sepsis requiring planned relaparotomy. Nowadays, OAT is used most frequently to prevent ACS after emergency abdominal surgery for trauma, ruptured abdominal aneurysm, or abdominal visceral disease. The other main indication is the treatment of IAH or ACS. In most instances, it is a midline laparotomy wound that is left open and requires TAC. Rarely, TAC is also required for abdominal wall defects after trauma or necrotizing infections. Fluid loss and heat loss occur but are manageable, and also protein loss is an important problem.1 Complications such as bleeding and fistulas arise frequently; the fistula incidence is related to the duration of the open abdomen treatment. Also, the choice of TAC technique may affect the complication type and rate.A further problem with pet hair is that it will clog up a vacuum cleaner cleaner hose if you pick up a hair ball or clump of pet hair at one swipe. The way to solve this problem is to vacuum cleaner slowly when using a hose with attachments. You should also vacuum cleaner slowly so that your vacuum cleaner cleaner is not "gulping" the hair in huge clumps.This technique was introduced to overcome the problem of fluid loss and incomplete barrier function observed in the first-generation TAC techniques. Abdominal drains were positioned in the gauze that covered the intestines and the wound was covered with an adhesive drape. The drains were connected to chest tube collectors, and although the resulting pressure was low (negative pressure of 20 to 25 CmH2O), they allowed easy control of the fluid accumulation under the dressing and prevented uncontrolled spillage (Fig. 4). Different names were used for this system that was described by Brock et al. for the first time; it has also been coined the Barker pack, VacPack, or Sandwich VacuumPack technique.2"4Equipment suppliers working in the photovoltaic industry are facing continuous cost pressures from end users. Simultaneously the production throughput, yield and uptime have to be improved to reduce the manufacturing cost per watt, which requires high performance transfer valves at the lowest cost possible.Nearly all pet hair finds itself on the furniture before long. The pet's hair can be discovered in the vehicle upholstery even when the pet is not allowed in the family vehicle.Early abdominal closure is therefore the ultimate goal of TAC, and the ICU care should also aim to facilitate this. Continued attention toward the prevention and treatment of IAH is imperative. Better insights in the pathophysiology as well as improved TAC techniques and surgical strategies allow primary fascial closure in the majority of the patients.Essentially the most significant attribute on a vacuum cleaner, if you enjoy dogs or cats that shed, is the type of brush roller. If your vacuum cleaner has a brush that revolves, you'll see that it picks up pet hair better than a vacuum cleaner that has no roller. Roller brushes are not all created equal.
